Habilitation Programs

(Program)

*Provides Habilitation Programs where all activities are goal-directed and based on the needs of the individual to remain independent within their own home. Occurs in a variety of settings with services including:
  • Community Habilitation
    • Budgeting money, grocery shopping, meal preparation, medical appointments.
       
  • Day Habilitation
    • Utilizes community establishments such as banks, retail stores, restaurants and volunteer sites for activities.
  • Day Habilitation-Senior Center
    • Transportation provided each day.
  • Residential Habilitation
    • Focus on daily living and/or hygiene skills
    • Community outings

Eligibility: Individual must be HCBS (Home and Community-Based Services) Waiver enrolled and diagnosed with developmental disabilities, Medicaid eligible. Age of eligibility varies based on service, call for information.

Application Process: Call for appointment.

Required Documentation: Checklist of documents provided during application process.
