* Assists families and caregivers of individuals with developmental disabilities in finding a break in their busy, everyday schedules.
Eligibility: 8 years of age or older. HCBS Waiver enrolled.
Application Process: By appointment, walk-in or referral.
Required Documentation: Show proof of being eligible for Medicaid and enrolled in Home and Community Based services.
Fees: Fees vary by program, Medicaid, Private pay.
