Consumer Directed Personal Assistant Services (CDPAP)

(Program)

* Provides services to assist those who are chronically ill or have a physical disability and want to live at home, but need skilled nursing services and/or help performing everyday tasks.
* Allows the flexibility and freedom for individuals to choose, hire, train, and supervise the staff/caregivers they invite into their homes.

Eligibility: Must have Medicaid or be eligible for Medicaid and be eligible for Home Care Services as determined by a doctor.
Individual must be self-directing, which means they are able to:
o Make choices about the way they want to live;
o Organize their environment to make these choices a reality
o Understand the consequences of their choices and accept responsibility for them;
o Assume responsibility for managing their own personal needs.
If they unable to be self-directing, they may designate a representative who is willing and competent to act on their behalf.
