* Helps individuals with mental illness and substance abuse issues connect with care they need to live more stable and productive lives. Screens for mental illness and substance abuse treatment readiness, refers to mental health and substance abuse treatment providers, assists in retention to behavioral healthcare and supports adherence to medical treatment.

Eligibility: Individuals who are HIV positive.

Application Process: Can do walk-ins but prefer to operate by appointment; call for information.

Required Documentation: Call for information.

Fees: No fees for service.

Herkimer, Madison, and Oneida Counties.
