Hospice & Palliative Care

(Program)

* Provides hospice, palliative and bereavement services to increase quality of life for individuals with a chronic or terminal diagnosis and their families in a variety of settings including homes, nursing homes, hospitals and the Siegenthaler Center 4 bed residence. Provides services to residents of Oneida, Herkimer and eastern Madison counties.

* Provides bereavement workshops and support groups as well that are open to the public and free of charge.

Eligibility: Terminal diagnosis, resident of area served.

Application Process: Call for information.

Required Documentation: Certification of physician of life expectancy of six months or less for Hospice services. For Palliative services at home patient must criteria as "homebound status" meaning they need a supportive device to leave their home.

Fees: Accepts Medicare, Medicaid, private insurance, do not turn anyone away due to inability to pay.

Herkimer and Oneida Counties, Eastern Madison County.
