Freedom Guide Dogs for the Blind

Provides a home-based style of training that allows Freedom Guide Dogs to bring a trained guide dog directly to the blind or visually impaired individual's home where they train with them for 2-3 weeks in their new environment.

Eligibility: Individuals 16 years of age and older, who are able to walk a mile daily and be able to travel independently, including crossing the sreet

Application Process: Call or go online to apply

Required Documentation: Proof of blindness, Physician and Ophthalmologist information, names and addresses of orientation and mobility instructors

Herkimer, Madison, and Oneida counties
