* Provides supportive housing for families that have a member diagnosed with HIV/AIDS.

Eligibility: 18 years of age and older.
Income must fall under the 200% poverty level
Veterans are given priority.

Application Process: By appointment.

Required Documentation: Must have documentation of HIV/AIDS status. Proof of housing vulnerability/history of homelessness.

Fees: Call for information.

Herkimer, Madison and Oneida Counties.
