* Provides supports and services to assist individuals who have sustained a traumatic brain injury with successfulÂ inclusion back into their community.

Eligibility: 18-65 years of age. Medicaid eligible.

Application Process: Contact Regional Resource Development Specialist/ARISE.

Required Documentation: Medical documentation supporting TBI occurrence/history.

Fees: Some private insurances can be accepted, TBI Medicaid waiver, and insurance contract negotiated.

Madison, Oneida, Jefferson, Lewis, Onondaga, Herkimer Counties.
