Eligibility: 14 years of age or older. OPWDD approval and have a developmental disability.
Application Process: By appointment.
Required Documentation: Proof of income and proof of developmental disability diagnosis before the age of 22.
Fees: Fees vary.
Herkimer, Madison Lewis and Oneida Counties.
