Maternal Child Program

(Program)

* Trained nurses provide home visits to pregnant women and parenting families.

* Provide assessment and education on a variety of health topics including health guidance.

* Referrals made to various community agencies as needed.

Eligibility: Resident of Madison County, prenatal women and parents of children up to 5 years of age.

Application Process: Call for appointment.

Required Documentation: None.
