Eligibility: Children from 20 mo. to 12 years of age depending on the service, resident of Oneida County.

Application Process: Call for appointment.

Required Documentation: Birth Certification, Social Security number, medical insurance card, current well-child physical, update immunization records. Pre-K: Proof of Utica residency

Fees: Call for information. Pre-K No fees for service.
