Open Doors Transition Services / Money Follows the Person

(Program)

* Provides transition specialists to help people move from long-term care facilities back into their homes and communities. A Transition Specialists can work with you and your family members to help you determine what services you will need to live on your own, as safely and independently as possible. We will also work with you to help you figure out how to get these essential services; such as:
  • affordable and accessible housing
  • the information and skills you'll need once you return home
  • any other additional services that you may benefit from (such as service coordination or case management).

Eligibility: Needs to have been living in a nursing facility, hospital, or Intermediate Care Facility/IID for at least 90 consecutive days (excluding Medicare covered rehabilitative care that is expected to be short-term in nature),
Needs to choose to move to a qualified community residence, such as A) A residence owned or leased by an individual or his/her family, B) A residence in the community in which no more than four unrelated individuals live, or C) An apartment with an individual lease with lockable access and egress, and which includes living, sleeping, bathing, and cooking areas over which the individual of his/her family has control, and
Must have received Medicaid inpatient services for at least one day prior to moving.
