Eligibility: Starting at age 3 and older. During the intake process, assist with OPWDD eligibility and applying for Medicaid.

Required Documentation: None.
Fees: No fee for services.
Onondaga, Oneida, Herkimer, Madison, Cayuga, Seneca, and Oswego counties.
