* Provides supplement assistance to low income households so they can obtain nutritious food to alleviate hunger and malnutrition. 

Eligibility: Must meet income guidelines.

Application Process: By appointment, walk-in or apply online https://mybenefits.ny.gov/mybenefits/begin.

Required Documentation: Proof of expenses, address and income.

Fees: No fee for service.
