Provides free commercial airline tickets to children and their families in need of specialized, distant medical care and/or second opinions. Flight programs include travel to United States treatment facilities and to train/retrieve a service dog.

Eligibility: Children in need of specialized, out-of-state medical care and second opinions who cannot afford transportation fees; call for income guidelines

Application Process: Submit completed application and necessary documentation (outlined in application package available online at www.miracleflights.org) via email, fax, or mail

Required Documentation: Doctor's certification form, letter from treatment site physician confirming diagnosis, waiver of liability, income certification, patient's birth certificate, current picture of the patient and brief description of the medical need
