Community Oriented Recovery and Empowerment Services

Assist individuals with serious mental health and/or substance use disorders meet their recovery goals such as finding a job, going to school, or socializing or living more independently.

Eligibility: 18 year or older, 65 year or older. Individuals must be enrolled in a managed care plan type (HARP or HIV SNP and have an H1, H4, or H9 code)
