Habilitation Programs

(Program)

*Assists individuals to access services in order to live full and independent life.

*Depending on an individuals interests and goals, linkage provided to services including:
  • Respite services. 
  • Habilitation programs. 
  • Employment services. 
  • Environmental modifications. 

Eligibility: Have a qualifying diagnosis through the Office for People with Developmental Disabilities (OPWDD) such as an intellectual disability, cerebral palsy, epilepsy, autism, a neurological impairment with onset prior to the age of 22.
Cannot be currently enrolled in other service coordination programs.
Cannot live in a residence that includes service coordination (nursing home, developmental centers, intermediate care facilities, etc.)

Application Process: Call for appointment.

Required Documentation: Proof of address, documentation of developmental disability.

Fees: Medicaid funded program.

Cayuga, Onondaga, Oswego and Madison Counties.

ARISE is an independent Living and community-based organization providing support services regardless of disability.
