Outpatient Rehabilitation

* Provides outpatient rehabilitation services at two locations. At the Faxton Campus, all services are available, and it is home to an aqua therapy pool and a rehabilitation gym. The second location is at St. Elizabeth Medical Arts, where physical therapy services are available. Available services offered between both sites include:

Lymphedema Treatment & management
Occupational Therapy
Pediatric Rehabilitation
Physical Therapy

Eligibility: Most services require referral from physician or extender.

Application Process: Most areas are by appointment only call for information. Hearing Instrumentation also has specific walk-in times in addition to appointments.

Required Documentation: Prescription needed for most services.

Fees: Fees and costs vary. Insurances accepted for therapy areas.
