In-Home Services; Health Supportive Services Program: Consumer Directed Personal Assistance Program

* Gives participants the flexibility to hire and train their own personal assistant to help with home health care needs. Services include:
- Eating.
- Bathing.
- Grooming.
- Dressing.
- Personal Hygiene.
- Cooking.
- Shopping.
- Laundry.
- Cleaning.
- Household Chores.
Eligibility: Receive or be eligible to receive Medicaid Need assistance with daily living tasks in order to live independently. Need long-term assistance (over 180 days).
Application Process: Call for appointment.
Required Documentation: Insurance card.
Fees: Medicaid, call for information.
Cayuga, Madison, Oneida, Onondaga, Oswego and Seneca Counties.
