Ombudsman Services

(Program)

* Working to resolve problems and improve the care of individual residents, while working to bring about change to improve care. AN OMBUDSMAN:
  • Will listen to residents' concerns, comments, and opinions.
  • Will advocate to ensure the highest quality of care for residents of long term care facilities.
  • Will work to resolve conflicts between residents and the long term care facility and help to develop collaborative solutions and resolve resident related concerns with the long term care facility.
  • Will investigate complaints and make appropriate referrals.
  • Will analyze, comment on and monitor the development and implementation of Federal, State and local laws pertaining to the residents of long term care facilities.
  • Will educate residents, family members and staff about resident’s rights and available resources.

Eligibility: Resident of a long term care facility, or legal guardian of a resident in a long term care facility.

Application Process: Call for appointment.

Required Documentation: Authorization from resident for program to resolve concerns.

Fees: Free service to local long term care residents.

Herkimer, Madison, Otsego and Oneida Counties.
