Offers comfort care for people with terminal illness and support for their families at home, nursing home facilities, hospitals, Francis House, or assisted living facilities.

Eligibility: Individuals in Cayuga, Madison, Onondaga, and Oswego counties with a life expectancy of 6 months or less.

Application Process: Call for information and services.

Required Documentation: Documentation of illness and terminal prognosis.

Fees: Covered by most insurances, no one refused for inability to pay.

Hospice of Central New York and Hospice of the Finger Lakes

Hospice of Central New York and Hospice of the Finger Lakes provides end-of-life care to patients and support for bereaved family members and/or caregivers of patients who have died under Hospice care. Hospice also provides support for non-hospice connected members of the community in need of services.
