Expanded In Home Care Services for the Elderly Programs

(Program)

* Provides individuals the connection to assistance necessary to stay in the home including:
  • Non medical personal care
  • Housekeeper/chore services
  • Case management
  • Information and referral services
*Allows individuals the opportunity to choose what services they receive through Consumer Directed EISEP.

*Provides linkage to outside agencies who will perform these services.
 

Eligibility: 60 years of age or older, resident of Oneida County, must be self directing for Consumer Directed EISEP. Participants in EISEP must not have Medicaid.

Application Process: Call for assistance.

Required Documentation: Call for information.

Fees: Sliding scale based on income.
