TeleCare

(Program)

Provides a daily friendly phone call or medication reminder to individuals who are isolated, homebound, and/or otherwise vulnerable.  Also provides temporary social support or medication reminders to individuals discharged from hospitals. 
 

Eligibility: Isolated, homebound, or otherwise vulnerable individuals
