Medically Supervised Outpatient

(Program)

* Offers evaluation/assessment services, referral, individual, family and group counseling and discharge aftercare planning.  DWI assessment and counseling.  Medication Assisted Treatment for Opioid Use Disorder including: Suboxone, Vivitrol, and Sublocade. Treatment of pregnant women using Subutex. Integration of care for those patients that are diagnosed with co-occurring disorders as well as substance abuse.

 

Eligibility: 13 years of age and older.
Must meet criteria for diagnosis of substance abuse or dependence.

Application Process: Walk-in for assessment.

Required Documentation: None.

Fees: Medicaid, Medicaid Managed Care, most insurances/HMO's, self pay (based on a sliding fee scale).

Herkimer, Madison and Oneida Counties.
