Surrogate Decision Making Committee

* Administers the Surrogate Decision Making Committee programs that aids in making medical decisions for the disabled.

Eligibility: New York State Run by Peacemakers in certain counties for people with intellectual, developmental, mental health, alcohol or substance abuse disabilities unable to make their own decisions or who do not have a relative or legally authorized surrogate willing to make the determination on their behalf. Eligible individuals are or have been in a residential program or received services from a program operated, licensed or funded by the Office for People with Developmental Disabilities (OPWDD), the Office of Mental Health (OHM), or Office of Alcoholism and Substance Abuse Services (OASAS).

Application Process: Must be referred by the New York State Justice Center for the Protection of People with Special Needs, see their website.

Fees: No fees for services.

Broome, Cayuga, Chenango, Cortland, Delaware, Herkimer, Lewis, Madison, Oneida, Onondaga, Oswego, Otsego, Tioga, and Tompkins Counties.
