Eligibility: Contact physician and ask them to fill out the Eat Right, Live Right Referral Form, making sure the form includes a complete diagnosis and a doctor’s signature.

Application Process: By appointment.
Required Documentation: Referral from doctor.
Fees: For individuals without coverage for nutrition counseling, there is a self-pay option, which can include a 5-month payment plan or be paid in full. Individuals may apply for the financial assistance program that is based on household size and income.
