In Person Assistor/Navigator Program

* Helps individuals, families and small businesses learn about and apply for health insurance, including Medicaid, the Essential Plan, Child Health Plus and qualified health plans.

Eligibility: Can't be Medicare eligible.

Application Process: By appointment.

Required Documentation: Proof of address, income, NYS Driver's license, Social Security Card, Birth Certificate, Immigration documents, current health insurance cards and bills, income documentation.

Fees: No fee for service.

Madison and Oneida Counties.
