* Operates 2 outpatient mental health clinics that provide critical support needed to understand and manage mental illness.

Eligibility: 18 years of age or older.

Application Process: Appointment required; call for information

Required Documentation: Proof of insurance, residency, and proof of income for sliding scale

Fees: Most insurances accepted, sliding scale fees if no insurance; call for information

Herkimer, Madison, and Oneida counties
