Eligibility: Ages 5-18. Must have Medicaid and Axis I Mental Health Diagnosis.
Application Process: By appointment.
Required Documentation: Proof of Medicaid insurance card and proof of Axis I Mental Health Diagnosis.
Fees: No fee for service.
