* Works with high-need women and families to access preventive and primary health care services, with an emphasis on pregnant women who are not enrolled in health care or supportive services.

Eligibility: Ages 13 and older.

Application Process: By appointment or walk-in.

Fees: Call for information.

Cities of Blossvale, Boonville, Camden, Rome, Taberg, Utica, Frankfort, Herkimer, Ilion, Newport, Ohio and Poland. Can serve other areas of Oneida and Herkimer Countie. Call for information.
