Johnson Park Apartments for Chronically Homeless Single Women
(Program)
Provides supportive housing for women with a diagnosed mental illness, substance abuse, or dual diagnosis. Services include case management, preventative services, life skills training, medical and mental health services, vocational and job training, education and employment opportunities.
Eligibility: Women ages 18 years and older who meet income guidelines and have: - mental health diagnosis or dual diagnosis substance abuse - significant risk of homelessness
Application Process: Appointment required; call for information
Required Documentation: Proof of expenses, substance abuse/mental health diagnosis, homeless documentation; call for information
Fees: Rent/fee secured through DSS; call for information
