* Provides comprehensive preventive, diagnostic and treatment services for individuals who have any of a variety of conditions in which they have difficulty falling or staying asleep, abnormal behaviors during sleep or difficulty staying awake.
Eligibility: 18 years of age or older.
Application Process: By appointment.
Required Documentation: Physician referral.
Fees: Most insurances accepted.
