* Provides, information, referral and follow-up for families of children who have or are suspected of having a serious or chronic physical, developmental, behavioral or emotional condition.

* Information and referral services are provided to help families access community resources.

Eligibility: Under 21 years of age, concern of having a serious physical, developmental, behavioral or emotional condition.

Application Process: Call for appointment.
