Placement Assistance Program

(Program)

Provides a referral for a Patient Review Instrument (PRI)/Screen to determine appropriate level of care for individuals interested in entering a skilled nursing facility.

Eligibility: Resident of Oneida County, 60 years of age and older, client must be willing to enter into a skilled facility and have already applied to intended facility, must be able to identify funding source or have applied to Medicaid.

Application Process: Call for a referral.

Required Documentation: Call for information.

Fees: Accepts donations.
