* Designed to enable individuals with chronic substance abuse issues to obtain and remain in safe, affordable, permanent housing. Program components include:

Rental subsidies
Program supervision
Counseling services

Eligibility: 18 years of age or older. Must be at risk for homelessness and have 2 inpatient hospitalizations or 5 ER visits within the past 12 months.
