Guardianship Services

(Program)

*Assists families with the process of procuring formal guardianship for their family member with developmental disabilities.

Eligibility: Individuals with a developmental disability, age 18 years or older with OPWDD or Family Support Services (FSS) eligibility.

Application Process: Call or walk in for assistance.

Required Documentation: Coordinator works with family to gather necessary legal documents.

Fees: No fee for services.
