- Falling, balance, and mobility problems
- Dizziness
- Incontinence
- Increasing memory loss, confusion or disorientation
- Multiple chronic illnesses, especially if there are multiple medication and/or doctors
- Persistent problems with mood, motivation, or sleeping
- Chronic pain or discomfort, including arthritis
- Increasing problems with self-care, and living independently
- General frailty
- Complete evaluations of the medical, functional, cognitive and psychosocial problems, and abilities of the older adult
- Help in coping with the health problems and social needs associated with aging
- Ongoing medical follow-up and case management for patients with dementia
- Education, counseling and guidance for families, caregivers, and other professionals
- Collaboration with the primary care physician and other health care providers
Eligibility: Ages 60 and over.
