(Program)

* Provides treatment of the following conditions:

Falling, balance, and mobility problems
Dizziness
Incontinence
Increasing memory loss, confusion or disorientation
Multiple chronic illnesses, especially if there are multiple medication and/or doctors
Persistent problems with mood, motivation, or sleeping
Chronic pain or discomfort, including arthritis
Increasing problems with self-care, and living independently
General frailty

* Offers the following services:

Complete evaluations of the medical, functional, cognitive and psychosocial problems, and abilities of the older adult
Help in coping with the health problems and social needs associated with aging
Ongoing medical follow-up and case management for patients with dementia
Education, counseling and guidance for families, caregivers, and other professionals
Collaboration with the primary care physician and other health care providers

Eligibility: Ages 60 and over.
