* Provides the following services for adults and children with multiple chronic health conditions, HIV/AIDS or a persistent mental health condition. 
  • Care Management
  • Transitional care
  • Patient and family support
  • Referral to community services

    Eligibility: Must have two or more chronic medical or mental health conditions, or one of the singly qualifying conditions. Must be enrolled in Medicaid. Children's Health Home: Birth to age 18 years. Adult Health Home: Age 18 or older.

    Application Process: Call for information.

    Required Documentation: Call for information.

    Fees: Medicaid and Medicaid Managed Care. No fee to enroll.

    Herkimer, Madison, and Oneida Counties.
