Children's Health Home Care Management

(Program)

Provides case management services to children with serious mental disturbance or chronic health conditions to assist them in staying healthy, out of the emergency room, and out of the hospital.

Eligibility: Children with: 1) Active Medicaid 2) A diagnosis of serious emotional disturbance OR two or more chronic conditions

Application Process: Call for information and services

Required Documentation: Medicaid ID card and documentation of diagnoses

Broome, Cayuga, Chenango, Cortland, Madison, Oneida, Onondaga, and Oswego counties
