Hospice of Central New York and Hospice of the Finger Lakes

Hospice of Central New York and Hospice of the Finger Lakes provides end-of-life care to patients and support for bereaved family members and/or caregivers of patients who have died under Hospice care. Hospice also provides support for non-hospice connected members of the community in need of services.
Eligibility: Patients and bereaved family members and/or caregivers of patients who have died under Hospice care in Cayuga, Madison, Onondaga, and Oswego counties
