* Working to resolve problems and improve the care of individual residents, while working to bring about change to improve care. AN OMBUDSMAN:

Will listen to residents' concerns, comments, and opinions.
Will advocate to ensure the highest quality of care for residents of long term care facilities.
Will work to resolve conflicts between residents and the long term care facility and help to develop collaborative solutions and resolve resident related concerns with the long term care facility.
Will investigate complaints and make appropriate referrals.
Will analyze, comment on and monitor the development and implementation of Federal, State and local laws pertaining to the residents of long term care facilities.
Will educate residents, family members and staff about resident’s rights and available resources.

Eligibility: Resident of a long term care facility, or legal guardian of a resident in a long term care facility.

Application Process: Call for appointment.

Required Documentation: Authorization from resident for program to resolve concerns.

Fees: Free service to local long term care residents.

Herkimer, Madison, Otsego and Oneida Counties.
