Eligibility: Must be living in a nursing home for more than 60 consecutive days. Wants to live independently in the community and can do so safely. Are moving to a qualified community residence such as: owned or rented home, residence in the community with no more than four unrelated individuals residing at, rented apartment with an individual lease. Are a Medicaid recipient.
Application Process: Call for appointment.
Required Documentation: Call for information.
Fees: Paid through Medicaid.
