Adult Health Home Care Management

(Program)

Provides case management services to adults with chronic health conditions, serious mental illness, or HIV/AIDS to assist them in staying healthy, out of inpatient care, hospitals, and emergency rooms.

Eligibility: Individuals must: 1) Currently have active Medicaid 2) Have two or more chronic health conditions, OR be living with HIV/AIDS, OR have a serious mental illness

Application Process: Call for information and services

Required Documentation: Medicaid ID card and documentation of diagnoses
