Eligibility: A referral from your healthcare provider is required and CNY Diabetes can obtain the referral for you if necessary.
Application Process: By appointment.
Required Documentation: None.
Fees: Insurance accepted. The program is Medicare-approved, and many other insurances also cover diabetes education.
