Application Process: By appointment. This location is only open to patients of the MVHS Medical Group.
Required Documentation: Proof of pregnancy to schedule first OB appointment. Only when working with Account Specialist to help obtain Insurance coverage: Proof of expenses, Proof of address, Proof of income.
Fees: Majority of insurances taken to include Medicaid, Medicaid Managed Care and Medicare
Herkimer and Oneida Counties.
