Provides support to individuals living with HIV / AIDS in navigating complex health and social challenges. They have dedicated care managers who collaborate closely with clients to develop individualized care plans that ensure access to essential services and fostering overall well-being.

Eligibility: Individuals who are living with HIV or AIDS

Application Process: Can do walk-ins, but prefer to operate by appointment; call for information

Provides an array of support services to individuals affect by chronic diseases, including HIV/AIDS, diabetes, heart disease, obesity, asthma, substance abuse disorders, and serious mental illness, as well as nagivators to help individuals and businesses make health insurance decisions.Offers a Syringe Exchange, and provides a variety of prevention and sexual health services to individuals ranging from youth through adulthood.Coordinates a network of Q centers and offers safe places for lesbian, gay, bisexual, transgender and questioning youth, their families and allies and offers support groups and seeks to promote an inclusive community.
