Home Based Behavioral Health Services

(Program)

* Provides behavioral health services to an individual with an intellectual or developmental disability in their home. Program includes:
  • Assessment
  • Development of a Behavior Support Plan for the Family
  • Training on the plan

Eligibility: Individuals with a developmental disability of any age, enrolled in Home and Community-Based Services (HCBS). Individuals must live at home with a family member or with a caregiver and be Medicaid eligible.

Application Process: Call for appointment.

Required Documentation: Checklist provided during application process.

Fees: Call for information.
