Living Well with Chronic Conditions, Diabetes or Chronic Pain

(Program)

* Offers a six-week evidence-based workshops for those living with chronic pain, diabetes or chronic conditions. Led by two Peer Leaders, participants will learn how to better manage their health to lead more active and fulfilling lives.

Eligibility: Individuals with chronic conditions, chronic pain, and/or diabetes. Over 18 years of age.

Required Documentation: Referral from doctor preferred. Self referral also accepted.
