* Provides services for individuals who require assistance with daily activities such as dressing, bathing, and medication supervision. Other services and amenities include but are not limited to:
  • Access to nursing care.
  • Arrangement of primary care services.
  • Case management services.
  • Private & semi-private suites.
  • Three meals & snacks daily with restaurant-style dining.
  • Community living rooms.
  • Recreational activities.
  • Housekeeping and laundry service.
  • Personal care assistance.
  • Safety checks.

Eligibility: Must be independent with mobility and transferring, medically stable and continent or self-managed incontinence.

Required Documentation: Completed financial application and supporting documentation, health care directives: HCP, POA, Insurance information and completed Medical Evaluation from attending Physician (DSS-4449C).

Fees: Fees vary across residences. Long term care insurance, private pay, and supplemental security income (SSI) accepted. Call for more information.
